Photodynamic Antimicrobial Therapy abbreviated to (PACT) is a revolutionary approach to treatment of fungal toenail infections (onychomycosis). PACT treatment is a safe, painless, and effective treatment that destroys the nail fungus. The PACT treatment involves focussing a beam of high wavelength light on to the fungal infection.
The PACT approach utilises a combination of PACT Nail Fungus Gel and a PACT LED lamp. The affected nail is initially stained with blue PACT Gel, (photosensitiser) the gel is harmless to human cells and selectively targets fungi, making the fungal cell wall sensitive to the specific wavelength of light used by the PACT Lamp. The PACT Lamp then focuses a high wavelength of light on to the fungus to destroy the infection and to prevent future regrowth. As the PACT Lamp has only been used to target the fungal nail infection, there is no damage caused to your skin or nail.
The PACT treatment is suitable for those with a mild to moderate fungal toenail infection. Once the infection is deemed as severe, (infecting most of the nail plate / 3 or more nails / penetrating beyond the cuticle area, the effectiveness is drastically reduced. This is because there is a significant portion of nail (the nail matrix/root) which lies underneath and behind the cuticle, this nail cannot be treated with the PACT as the light cannot shine directly on this area of nail. There will still be parts of the nail not penetrated by the PACT therapy device.
If the nail matrix (root/cuticle) of the nail is involved the most effective treatment, subject to suitability is the oral antifungal tablet terbinafine available through prescription.
In order to determine the extent and severity of your fungal nail infection, our podiatrist would first conduct an assessment of the affected area. A visual diagnosis can usually be given if the distinguishing signs of fungal nail infection are identified, the treatment options available would then be discussed with you during your appointment. Occasionally a podiatrist may request a sample of nail is sent to a laboratory for further investigation to confirm it is fungal before commencing treatment.
During the first PACT appointment, the infected toenail will first be trimmed and filed by the podiatrist. The thickened nail will then be reduced using a podiatry drill. Doing this allows the PACT Gel to efficiently penetrate the fungal cells, facilitating a more effective treatment. Treatment on each infected nail will last approximately 20 minutes per session. The first 10 minutes of the session will consist of the application of PACT Gel to the infected nail; the following nine-and-a-half minutes will involve the PACT Lamp focusing beams of high wavelength LED light onto the infected area. The painless treatment allows you to continue with your activities of daily living as usual. This process is carried out 3 times within a 7-10 day period. A follow up appointment is recommended 4-6 weeks later, and then again 6-8 week after that, 5 sessions is enough for most people, but some may require further treatment. As with any course of treatment for an infection, it is imperative that this is completed. The healthy nail(s) should then begin regrowth infection free. It can take 12 to 18 months for a whole toenail to regrow, although nail growth rates do vary from person to person.
To prevent reinfection, it is important that you take the utmost care of your feet. The podiatrist will discuss an aftercare plan in detail which must be followed throughout and after the treatment package.
PACT treatment has been found to be free of side effects. Some patients have noted a slight tingling feeling during PACT therapy, but nothing that would cause pain. The PACT gel which is applied to the nail may cause a blue stain to the nail.
Patients are able to resume normal day to day activities directly after treatment.
