In respect to anatomy, a ‘sinus’ can be described as cavity/sac within the tissues. The sinus tarsi is a gap between two bones in the foot - the calcaneus (heel bone)and the talus, within this gap holds ligaments which stabilise the ankle. Sinus tarsi syndrome is the clinical syndrome of pain and inflammation at the sinus tarsi, which can be felt around the front/outer ankle.
Sinus tarsi syndrome most commonly happens after a traumatic injury to the ankle such as an ankle sprain. It may also be contributed from flat feet/overpronation, as this can cause irritation to the sinus tarsi. Occasionally sinus tarsi syndrome may be result of inflammatory conditions such as rheumatoid arthritis.

The first stage when treating sinus tarsi syndrome is to reduce inflammation which is the cause of pain. If the condition is proposed to be contributed from a biomechanical issue, this will be assessed through a gait analysis and biomechanical assessment. Orthotics may be prescribed to improved the function and foot mechanics, redistributing pressures of the foot and away from what may be irritating the sinus tarsi.
Podiatry for sinus tarsi syndrome would firstly include obtaining a thorough history, a personal account of the problem, an examination and a biomechanical assessment.
The purpose of a biomechanical assessment is to assess how the foot and the lower limb are functioning, the aim of which is to correct any abnormal biomechanics that are contributing to your painful symptoms with an orthotics.
